Here's a few things to keep in mind while we process your information
Send your NDIS plan to [email protected] (if you haven't already).
Endorse Plan Partners - if your current plan started after 30 October 2023 then you may need to endorse us. You can do this by contacting the NDIA on 1800 800 110 or your Local Area Coordinator.
Send a switch email to your current provider - if you're switching from another provider and your current plan started before 30 October 2023, then you may need to advice them that you are switching to Plan Partners. See example email below:
To whom it may concern,
I'm writing to let you know, I wish to exit your service for NDIS plan management/support coordination. I have completed a service agreement with another plan manager and would like to commence plan management/support coordination with them as soon as possible.
Please email me back to confirm you have received this request, stating the date you will release my service booking.
